Position Description
The caregiver provides medical and physical care to the residents of the household to meet their daily needs and enable them to function at the highest possible level. While this care is provided under the leadership of a professional nurse, the caregiver is the residents’ closest advocate in the household. The caregiver promotes the residents' psychosocial well-being through meaningful relationship building, and works with the household team to meet other social service needs of the resident and of the household, always providing support, friendship and kindness for each of the household members. The caregiver plans and facilitates social activities for residents of the household which provide meaningful ways to spend time as recreation, exercise, relationship building and fun. The caregiver also supports the household homemaker in maintaining a clean safe household environment and in caring for the resident’s personal laundry. All caregiver tasks are performed in accordance with current standards, guidelines and regulations and in a safe and efficient manner.

- Reports to and is supervised by the Household Coordinator, LPN & Clinical Coordinator

General Duties and Responsibilities
1. Attends to all resident personal hygiene and personal care needs, including but not limited to grooming, hygiene, dining and nutritional, mobility, elimination, psychosocial and safety needs. Organizes all supplies necessary to complete the tasks. Supports residents in self care, and provides care as necessary for those unable to care for their own needs. Follows universal precautions, proper infection control, sanitation and safety standards of practice in all work and activities
2. Provides restorative therapy under the leadership of therapy professionals and in accordance with the resident’s functional maintenance plan
3. Observes residents closely, identifying changing needs and conditions at first indication, and reports immediately to nursing or household leadership as appropriate
4. Participates actively in the resident’s care planning and care conference, serving as the residents' closest advocate in directing their care and honoring their preferences
5. Consistently works with all household team members, led by resident and family preference to meet the needs and desires of all household residents.
6. Continually seeks opportunities to meet resident’s psychosocial needs through both care planned and spontaneous actions
7. Continually seeks opportunities to support the social needs of all members of the household through support, friendship and kindness
8. Plans and organizes social activities for individual residents, small groups and for the household, making any necessary arrangements to assure success of the activity
9. Seeks opportunities throughout the day to facilitate spontaneous activities of resident choice and household preference; accompanies resident in all activities of choice
10. Leads and participates in learning circles with residents and staff in the household daily
11. Maintains equipment, games, supplies for use in supporting resident and household activity choices
12. Continually seeks opportunities to share personal interests and talents with members of the household through both planned and spontaneous activities
13. Maintains appropriate documentation in support of social service and activities functions in the household
Ancillary Duties: Housekeeping
1. Maintains clean spaces in the household, including resident rooms, household common spaces, as well as support spaces and equipment
2. Maintains supply of all materials to meet resident needs in the household
3. Supports all aspects of the household environment, including pets, plants, children, visitors and guests, facilitating all planned and spontaneous needs and activities
4. Coordinates work with the household homemaker to ensure the housekeeping needs of the household are met within the staffing resources of the household
Other Duties:
1. Performs any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor.
Typical Physical Demands
1. Employee must have the ability and knowledge to lift and move residents safely.
2. Must be able to lift 30 lbs. of weight; to reach and extend hands and arms in any direction; to seize, hold, grip, turn or otherwise work with hands.
3. Be able to bend, stoop, kneel, bend at the waist.
4. Be able to clearly hear sounds as is needed in monitoring blood pressures.
5. Be able to see clearly at 20 inches or less.
6. Have an acute sense of smell.
7. Must be able to push 200 lbs. and transport residents in a wheelchair between buildings.
8. Must be able to sit and stand for extended periods of time.
9. Must be able to successfully complete the WorkSTEPS Functional Testing program. This program assists in determining whether applicants/employees are able to safely perform essential job functions.
Qualifications
Key Knowledge, Skills, and Abilities
1. Be able to express verbal ideas and impart information to residents and residents’ families.
2. Extensive ability to respond to emergencies rapidly.
3. Must be able to operate Hoyer Lift and Arjo whirlpool tubs.
4. Work habits should include regular attendance, punctuality, teamwork, initiative, willingness to learn, dependability and promptness.
5. Must be able to perform mental functions and withstand moderate amounts of stress.
6. Must be able to work flexible hours to meet the demands of a 24 hour daily operation.
7. Strong verbal communication skills. The ability to read, to write, and to speak fluently and correctly in the English language.
8. Ability to embody and uphold the corporate personality and culture of Arbor Acres with its emphasis on warmth, caring, generation of community, enthusiasm, achievement of excellence, and joy.
9. Completion of cross training for household activities, social service, housekeeping, laundry required.
10. Completion of cross training for household homemaking, including food service and food handling (ServeSafe or equivalent) desired.
11. Ability to read, write, follow oral instructions, and report progress
12. Completion of approved nurse aide training and competency evaluation program.
13. Currently listed and in good standing with the DHHS HSR Nurse Aide Registry.
14. Previous nursing service and long-term care experience preferred
15. Ability to recognize changes in resident status and ability to convey changes to the staff nurse and/or nurse supervisor
16. Genuine interest in geriatric care
17. Understanding, patience, tact in dealing with the chronically ill.

Minimum Education
1. High School Diploma or GED preferred, but not required.
Minimum Experience
1. Prefer 6 months experience.
License and/or Certification Required & Continuing Education Requirements
1. Certified as a Nurse Aide I and registered with the North Carolina Department of Facility Services Nurse Aide Registry.
2. Will comply with all federal and state regulatory requirements to maintain certification and active registry as CNA required.
3. Completion of requirements to maintain food handling certification (ServeSafe or equivalent) desired.
